Harlequin Crest, also known as “Shako”, is a unique helmet in Diablo II Resurrected. It can be equipped by all classes provides huge bonuses to life, mana, +skills, big magic find and even damage reduction. If you want to know how to get it as fast as possible and if it fits to your build, stick with this guide until the end. The Harlequin Crest is worth searching for, as it has awesome bonuses, that make up for the quite low armor rating. It gives you pretty much everything you want and is a very popular item for many rich and spezialized builds.
This is already the elite version of this Helm, so it cannot be upgraded any further. The normal version is called Cap, while the exceptional version is War Hat.
Note: If this item can have sockets, you might be able to get some runes and get one of the runewords as a result - if you place them in the correct order.

Every Act Boss can drop The Harlequin Crest, but these are the ones with the highest chances. Especially Baal and Mephisto have a good chance to drop the Shako. So if you want a more challenging fight, don’t hesitate to farm with Mephisto. If you are struggling with him, simply farm the other bosses or areas, or read our guide on how to beat him.
Mephisto’s chance of dropping Shako is around 1 out of ~2000. You can also find it in the secret cow level. Although The Cow King drops the Harlequin Crest / Shako only pretty rarely, the chances are decent if you are killing Rakanishu and Griswold on Hell Difficulty.
